What is "the info"?

for normal distribution of test scores, about 95% fall between 38 and 62, 2.5% are below 38, and 2.5% are above 62

For mode look at that Gaussian distribution curve and you'll see that the probability is maximal in the middle, so at a score of 5, because your distribution is centred around that value. For standard deviation you should look where the normal distribution assumes those 2.5% and then normalize to match your -12 / 12.
