Find the slope by taking the difference in y over the distance in x. Then plug into point-slope for one point and use algebra to convert to slope intercept.

Well, you have the y intercept there with the second point, then you can find the slope by taking the change in y over change in x. The slope-intercept form is y = mx + b where m is the slope and b is the y intercept (at what y value does the line cross the y axis - in other words what is y when x is 0).
