How do organs get the nutrients from blood?

Usually there is a positive concentration gradient of nutrition between blood and organs. Most of the nutrition just follow the law of diffusion, they diffuse from blood to other tissues. But for some other nutrients there are some protein on the memberane of the cell, they actively transport nutrients from blood to tissue.
