MLA is usu. used for Humanities Disciplines. It is used for  Document Format, Text citations, Works Cited, or a list of all sources used in the paper. It is also necessary to follow your teachers guidelines. Although headings in papers are outnumbered afterall they are optional.
As it is necessary to follow the instructions of your teacher as well, whereas in your case you should write in the top left corner the following data,
Your full name.
Your teacher's name.
The class/period.
The date.
And the formal font size is given 12 for The Roman Times, if you are formatting it on Word.

Where as the title page (separate), is not necessary unless the instructor specifically asks for one.

That varies depending on who is accepting the document. I'd ask them to be sure.
