Is it required to put the prefixes in formulas? Like Ca(OH)2 --> Calcium hydroxide or Calcium dihydroxide?

Technically yes, especially for things like carbon monoxide and carbon dioxide. Some chemicals have a formal name like calcium dihydroxide but have a common name like calcium hydroxide.
