what is insanity defense? what standards have to be met before a person is judged insane? do these standards differ from one state to another?

As my understanding would have it, the insanity defense usually calls for cognitive insanity whereby a person is afflicted in such a manner that they are unable to make moral and correct decisions. This is the more widely used insanity plea however this is also a second called plea termed volitional insanity which states the offender was in a perfectly fit state of mind but the urge to commit the crime was too great to deny. And any person claiming insanity would be met with a battery of examinations, of which I do not know the names, that set the standard for mental health. And I am also unaware of the variations between the states though I am certain many exist.
