equation for the line is \[y-\frac{1}{\sqrt{2}}=\frac{1}{4\sqrt{2}}x\] or \[y=\frac{1}{4\sqrt{2}}x+\frac{1}{\sqrt{2}}\] feel free to rationalize the denominators
