Sometimes when someone answer's questions you get notification of receiving medals instead of saying that there is new answer.
I haven't tested it but it might be, that when user gives you medal and IF he answers it again it's the case when you get notification of receiving a medal instead of answer.

yeah it's probably when it happens, i just answered question, then user gave me medal and as always I received notification of medal, but after few second same user also answered to that question and i received notification of medal again instead of notification of answer
