I'm writing a research report. Should I include the questions I ask in the study in the methods section?

Yes. I would think so. If it is a long survey you could add it as an appendix and refer to it
"The complete survey can be found in Appendix 1." If there are only a few questions, you could make a table and insert it into the narrative.
