(APA Style-based answer) If there is only one appendix, label it Appendix (centered), on its own page and place it as the very last item of your paper (after references). If more than one appendix, label with letters Appendix A, Appendix B, etc. In your paper, specifically refer to the Appendix in the appropriate portion of the text.
