Tissue rejection occurs usually in organ transplant. If the tissue does not match the recipient's tissue, the immune system will regard it as foreign and destroys it. A way to overcome it is to find the organ tissue very similar to the recipient's tissue so it can pass the immune system. It only reduces the chance of tissue rejection, but it can still occur

cmkc109 has given a good answer. However, the immune system can be damped down to reduce tissue damage by using drugs like FK506 / cyclosporin. The donor can also be treated with anti-rejection drugs - look up graft versus host disease if you fancy extra credit

Recieving or donating tissue to an Monozygotic twins can be done without any autoimmune response, The autoimmune response occures due to what is called a MHC (Multi histological complex) which all the cells in your body has (except Erytocytes). You immune system does not recognize the MHC in the transplanted tissue and consider it a pathogenic invader.
