Darn, there's no study group for setting up pediatric offices. Probably because this is not the place to be asking for professional help.

A handful of uni/college students (and a lot more high school/elementary students) may not help you with that.
I suggest asking the office who gave you the forms, one of those business-starting businesses (watch out, many sketchy ones about), or just your government.
