$$y=a(x-h)^2+k$$
here vertex is (0, 3)
so
$$y=a(x)^2+3$$
Now we need to find a, it's given that the parabola passes through(-4, -45)  . Substitute x=-4 and y=-45 
$$-45=a(-4)^2+3$$
subtract 3 from both sides
$$-48=a(16)$$
Divid both sides by 16
$$-3=a$$
So a=-3, The equation of parabola with vertex (0,3)  is
$$y=-3(x)^2+3$$

The part easily missed is "a" :
y = a( x -h)^2 + k
