What must happen for an inherited genetic mutation to result in a phenotypic change for a recessive trait? I don't understand who or what it would be inherited from..

I think this is something to do with autosomal dominance. the mutated gene needs only to be inherited from one parent. This is possible because the allele for the mutated gene overpowers the normal allele, though recessive, hence causing the disease.
