I'm writing my annotated bibliography and I have a couple questions.

Do I put my titles of my paper on the first page.

Do I put a header on my second page. If I do, it it: Last name, Page 2

And do I press enter in between sources, or just double space the whole thing?

Would you be able to tell us what citation format you are required to use? 
If the instructor did not specify a format, let us know the discipline. A determination can usually be made based on the universal format required in your area of study for your 3 questions. (I didn't want to just assume it is math, as in your name.) Thanks.

The site "bibme.org" creates the citation for you if you want.  Like imalinguist said, the citation will change based on the format
