OK, you are the first three correct. You can find the vertex by either completing the square, or using -b/2a=x and then solving for the y value by substituting the value you got for x back into the equation.

You dont have to answer the first 3 or last 2

I have to leave. But the answer to four is (2,-5) -8/2(-2)=2 So x=2 -2(2)^2+8(2)-13= -5
