Slope intercept form is the form it is currently in, y=mx+b, to find the intercepts you replace 0 for x and solve for y, then for y and solve for x.

For the first point you replace x with zero and get a result for y, which is your first pair (0,y) and when you do the same thing for y and solve for x you get your second pair (x,0) then you plot them.
