im writing a medical report about a patient who has bells palsy. i have the symptoms but when it comes to incorporating physical examinaton and the history of the patient it isnt coming together for me

If it is about a specific patient, it helps to follow a chronological order. You could start by describing the patient's overall metrics like gender, age, weight, etc. Then you could discuss the presenting symptoms. What tests or clinical protocols were run to differentiate Bell's Palsy from other possible diagnoses? Once diagnosed, how was the patient treated and what was the outcome in the case? Not that I write medical reports or case histories or anything.
