Hi I couldn't find it either, but I found a reference on wikipedia to Richard L. Burden, J. Douglas Faires (2000), "Numerical Analysis, (7th Ed)", Brooks/Cole. ISBN 0-534-38216-9. Have you read this book? Maybe it will be more enlightening as to the history of the method - books are sometimes more revealing of the history. Sorry I couldn't help you further.
