In a thesis/dissertation with a list of abbreviations, should all abbreviations be included, even if they are used only once and defined within the text?

Unless you're trying to save space, I'd say yes. Members of your committee (and other readers) may not read the whole thing. So, if they open to a section where you use the abbreviation, it's good for them to have a quick place to reference it. On the flipside, if you're only using the abbreviation once, then why use the abbreviation at all?

Fair enough, makes sense. Where I'm using it just once, it's where it's a commonplace abbreviation in the field and the abbreviation would be the more "usual" form to use than the fully transcribed name..

Ah. I don't think it matters either way.
