Good question. How....
I always like to pretend it was a mis-click and they meant to make someone else a mod and they just never got around to fixing it.
But I think the real reason was:
I been coming to OS a lot.
I followed the CoC ( I think I did; honestly I didn't know there was a CoC when i started coming here)
I just used common sense on my behavior.
I never asked to be a mod. I didn't talk much to the the admins. (So I know that is not a requirement.) I did talk to them a lot more when they made me a mod. Sorta had too. (Yucky, right? kidding of course-lol)
You do have to be noticeable though in order for them to consider you. Like being on top of the list in a popular group helped. So helping others helps you get considered.
I got along with many of the other users. (There will always be someone you don't get along with so it is not required you get along with everyone, but do your best.)

Maybe there was more they considered that I'm unaware of...

But this is all that I know...
