When combining two substances to form a mixture, do you think any form of chemical reaction is taking place? Also, if they are forming a mixture would they be separate or mixed between each other?

As per the definition of a mixture, there should be no chemical process occurring, so you can eliminate option A.

So i guess am going with option B...because it has to interact correct?

Correct. It wouldn't be a very good mixture if the two substances were still physically separated.
