Patient: Ridge Hunter Account number: 994789118743 DOB: 12/11/83 Sense of humor pancreatitis Mr. Hunter is an 80 yr old male his height is 5’3 and weighs 137 pounds visits the Emergency room for complaints of nausea and pain in throat. Mr. Brown’s medical history suffered from acute pancreatitis. Mr. Hunter’s grandchildren have suffered from some history of medical problems. Mr. Hunter’s granddaughter suffered from cystic fibrosis and grandson suffered from chronic gallstones. Patient explains his pain on a scale 1 to 10 he says it’s a 7. Patient said the pains begin a week ago and has gradually become worse by the day. Mr. Hunter’s prognosis was not getting any better. The nurse performed an endoscopy on Mr. Hunter and the nurse was alarm to found hyperlipidemia and hypertriglyceridemia which was unbelievable. The nurse documented this then decided to listen to the patient’s sounds with a stethoscope. The nurse demanded to have a CAT scan on Mr. Hunter. The CAT scan on Mr. Hunter shows that treatment is not going to work. Azathioprine and hypercalcemia is useless and the cause of Mr. Hunter’s pancreas to become very worse is the development of pseudocyst with the lack of insulin. There is no need for ERCP which will not have any kind of impact. The problem cannot be divisum. Lipase and Amylase were low so the nurse turned to insulin and furosemide. This resulted in not being any good for the patient. The nurse decided to give the patient azathioprine until the doctor diagnosis the issue. The doctor says the issue has caused the patient to have difficulty swallowing. The nurse gave a barium swallow test because the patient had a bone stuck in his throat. b

Oh yeah, I just saw, you have written CAT Scan instead. So Chris if you're thinking that this is wrong, as to relate all the diseases and symptoms and treatments, to only one patient, you might consider writing the same by associating different diseases, symptoms, and treatments to different patients.
