I have changed it a little.  I didn't put in all of y our "filler" information.  I only fixed the main outline, so you have an idea of where everything goes.  I hope I helped!
 

Your outline should have five parts (paragraphs).
I.	Introduction
   1.	Thesis
   2.	Three key points
   3.	Homelessness defined and characterized
   4.	Demographic characteristics of the homeless population

II.	Housing

III.	Healthcare and social services

IV.	Drug and alcohol counseling 

V.	Conclusion
   1.	Restate your thesis
   2.  Restate your three key points  	
   3.  Summarize.  Do not add NEW INFORMATION here.
