In an MLA formatted paper, if I mention the title of the book and the author that I am referencing in the beginning of the paper, and it will be ONLY that book/author throughout the entire essay, do I need to put the authors name in the parentheses of my in-text citations?

No, that is not needed in a paper using MLA format.
yes, dont take chances..you always need to put the authors last name no matter what.
