Ok he's saying they were basically really being screwed over, the answer is either c or d because, clearly he isn't saying they get the same treatment, he says that in the 3rd sentence, he also says that they suffer the cold.

sounds like c more than d because hes saying they are coughing up more money than disease... so i would go with c.
