The above named patient is under my medical care. He had a complete medical evaluation on 28/08/2012. I find him to be healthy and free from any infectious disease. Blood testing was normal. Hepatitis A. B & C, Syphilis, HIV, Gonorrhoea and Chlamydia testing were normal. PPD shows zero induration.
