How do you cite two references for one paraphrase?
Below is a paragraph I am having troubles with. Following the end of the paragraph are two sources I used and I don't know how to cite them both after the paragraph.
My Paragraph is:

Coronary artery disease, also known as heart disease, is a disease that develops when any of the coronary arteries become damaged or diseased.
Blah blah blah....

then at the end it has:

(Mayo Clinic Staff, 2012 and National Institutes of Health, 2012).

How do you cite two references for one paraphrase?
