Actually "Qué es su nombre" is completely wrong. The answer to that question is" Su nombre es la cosa que la gente usa para llamarle" Qué es = what is the definition of. His name is the thing that people use to call him. That's the definition. Cuál es su nombre? or Como se llama? are correct.
