1- In the middle ages of europe, the bible was in Latin/ or greek, and most everyone could not read. The pastors controlled the society by a scary amount, they could tell everyone exactly what they wanted, and no one could argue because they didn't know what to argue about! The people knew nothing, they had to trust in their pastors blindly. 

I can't answe the other questions right now, I've never heard of mixed life, but I'll see if I can find anything.
