You could either say silbato de violación or violación silbato. // The first one is more formal, probably more likely to be used as a newspaper title while the second one is more relaxed and would most likely be used more in the everyday Spanish community.

It definitely wouldn't be "silbato de violación" or "violación silbato". I haven't heard them called by a specific name, I don't think they are that common... You'd just describe it / explain that it's a silbato para alerta ante un ataque sexual / silbato para alerta ante acoso or something similar.
