if you ask a qustion, and there are people viewing that question, and u manually add an update, does that update automatically show up or do the viewers have to refresh the page ?

The update automatically. You should neither have to refresh nor leave the question and return. Everything is pushed in (near) real time to anyone viewing the question. People who aren't viewing it see the number of replies update in real time, etc, etc.
