I'm doing a project for a class in which we had to take a Leadership Potential Indicator assessment online and get results in a PDF. How would I cite this (APA)?

I've never used APA style citation. Unless teachers/professors don't specify a specific style, it's easier to just go with MLA. That website I gave you has examples for MLA, too.

Normally I go into microsoft word under manage sources and enter in all the information. Then when you go to make a works cited page it neatly organizes it for me. I don't know if you find this helpful. If not then google purdue owl APA citations. there is always helpful information on their website.
