Jus manipulate algebraically until you get it into y = mx + b form. From there, m is the slope, and b is the y value of the intercept. (y intercept is (0,b))

Subtract 12 from both sides, then add 3y to both sides. After that, divide everything by three to get it into slope-intercept form.
