Is it "Excuse me sir?" or "Excuse me, sir?"?

It mostly depends on your tone. If you are talking in a more angered, or agitated manner, it would be "Excuse me, sir?", but if you are asking in a more questioning way, it would be "Excuse me sir?" I myself would go with "Excuse me sir.".
