I believe it's different in every country, cuz in America, it's not necessarily an exam, but more of an interview and presentation on your research. During the interview is where the exam is since you are asked detailed questions about your project and also submit a long research paper which is often published. At least that what I've heard from colleagues. I have not applied to graduate school yet.

Unless you are talking about the GRE (Graduate Review Exam) which in that case I know more about. And I will let you know that it is EXTREMELY difficult.
