Question 4. should be:
Tu nous as écoutés à la radio? (4 points)

which makes the answer easier to find.

Question 5 should be:
Est-ce que ta mère porte ses lunettes tous les jours pour travailler?

agree with vincent for the question 4, not the 5 it's not a typo if u speak of a pair of glasses in particular
Tu nous as écoutés (on the radio): "nous" means that not just one people was on the radio, but at least 2: i would answer:
Oui, je vous ai écoutés
Answer 5 is Oui, elle les porte tous les jours (yes, she wears them everyday). In the question it's written "LES lunettes" so "LES" must be in the answer, forget L' and LA. Saying "elle porte les tous les jours" is a nonsense in French. (it sounds english actually)
