I'm designing a web form, can anyone tell me how to import passport and bio-data please.

Use Google Docs. You can create forms on there and either embed them into a webpage you're building (and style them to an extent using div id's in your CSS code) or you can even email them. They can be posted to Facebook as well...in case you ever need a survey done for any reason
