y = mx + b is the slope-intercept form of the equation of a line, where m = slope and b = y-intercept. Just use the values you have for slope and y-intercept in the equation above.
so it's y = mx + b

Replace m with the m you are given, and replace b with the b you are given.
You are told slope = m = -5, and y-intercept = b = -1. Use those values to replace m and b in the slope-intercept equation.
