what type of fossil is a tooth fossil?

I believe it is referred to as a TRUE FORM FOSSIL.

yes they are true form fossils (which means the fossil is a whole body or body part). teeth are also known as BODY FOSSILS - because they come from an actual body part (other examples are:, bones, scales, claws) . . . as opposed to TRACE FOSSILS - which are traces left behind by the animal, (such as bite marks, footprints, etc.). body fossils tell more about the animal's size and shape, while trace fossils tell more about it's behavior. there are many ways that fossils are formed.
