Umm, be a little bit more specific. There are a lot of reasons a hard disk might fail. Also, do you mean fail as in going offline? Or as in becoming completely nonfunctional?

Actually, a crash is a specific type of failure and it is literally what happens. In a hard disk, the head is known as a flying head. It literally flies over the disk, never touching, and air flow dynamics in closed systems are involved. However, as the drive wears there is more vibration in the system. This can cause the head to touch the disk, damaging the head, disk, or both. The technical term for this type of damage is a hard drive crash.
some times I hear like, due to mal function of software all data is lost due to HD crash. Why it occurs.
They are misusing the term. People like to say crash all the time, sounds impressive. However, a crash is only one type of hard drive failure. A program crash is when a program "takes a nose dive" to use a phrase related to the same thing. If a program has an error and it locks up the machine, you may be forced to shut down the machine improperly. Any time the machine is not shut down properly, there is a chance for data loss or for things to be written to the wrong part of the hard drive. This second thing, a program crash, does NOT cause a drive crash. But because both have the word crash, some people confuse them. That, with the popularity of something as dramatic as the word crash, makes people misuse both terms.

A stop error can be caused by defective RAM, an overheating CPU, software issues, and more. Formatting only addresses most software issues. If it is a hardware issue or a software conflict, then formatting will do nothing. the hardware will still be defective and you will be reinstalling the conflicting software. Every stop error on every computer is a unique situation. It needs to be properly evaluated by an experience person. Or, a person can gain experience by researching the error messages given and finding solutions. There is no magic method around this.
