I'm gonna fill out the form :-)
I already have the full design of the mobile app worked out a while ago, so I'm just gonna submit it and describe the design.

Could I also submit my existing code for the app as a supplement?

BTW You can leave me out of the money part of it if you wish :-) I don't need it. :)

So is it a native app (if so, for which operating system? each mobile OS has different design rules), or is it a web app ?

I worked out mine for iOS, and it's a web/clip app :-)

I could also do a native app BTW

Dumb, we are looking for photoshop/even hand drawn mockups. We want to get the concept. I dont want the app -yet.
