well, just plug values for "x" and get "y", and stick them in the cartesian plane

\(\bf y = \sqrt{x-4}+1\\ \text{x = 4}\\ y = \sqrt{4-4}+1 = 1\\ \text{so (4, 1), now x = 29}\\ y = \sqrt{29-4}+1 \implies \sqrt{25}+1 \implies 5+1 = 6\\ \text{so (29, 6)}\)
