I think you can't access webcam in php since it's server sided, you need client sided language. You'd need a plugin like Adobe Flash to do this.
so in case of hangout and skype which style did they use to access the webcam and microphones ? @thomaster

Also aren't skype and hangout programms that you need to download by the way? And not in browser? But I know tinychat has a webcam feature while it's available in the browser so it is possible.
actually i thought that the webcam can be accessible(so when doing video calling it is just the matter of exchange by IP address)

No I don't programm that much. Actually I'm a biologist :P I used to do computer science but I never used video calling technology

Alright! My head is getting sick already of all the lies and slander in this post. :P Anyways, of course this has to be done on the client side (obviously, everything displayed to you, even watching a video has some client code), but Flash is a method, correct. Flash, and similar plugins were the only way for quite a while- until Web RTC. RTC allows you to use codes to utilize video transferring directly from the browser, no need for any external plugin- and furthermore, it's much, much better quality. Ofc these things like Video chat with these technologies can become very hard to code from scratch, so using an API like http://opentok.com/ would prolly be a good idea. I've used them before: quite amazing. You can pick Flash or RTC (they show both of their benefits) and from then on, they have code samples and an API for creating sessions. Note that as far as I know, you need to utilize a server language (that's where something like PHP would come in) to create sessions. Tell me if you need any further help :-)

Tokbox certainly has an incredibly open API, but theyre laggy and prove to have a lot of reverb if more than two users are in the session. Facebook uses Skype, and that requires adobe flash.... However, there are enormous strives towards a completely HTML5 based video implementation system, which would completely revive the clumsy functionality of adobe flash and honestly change the way we interact. All we have to do now is wait until browsers join the bandwagon. For now, TOKBOX is where its at. Other than that, you're looking at other company logos or an enormous development bill.
Yeah, true @gschindl Though Tokbox only mainly lags a hell when you go with it's Flash mode.
