How do i find the equation of a line that passes through two points?

Put the coordinates of the points into this equation (y2-y1)/(x2-x1)

That will give you the slope

Once you find the slope graph it or make a table starting from one of the original points and see when x is 0 what is y. The number y is the y intercept which is the second part of your equation
