I am not 100% sure about this...but I am gonna put what I think. You have to pay the $200 deductible before the insurance kicks in. And you insurance pays 75% of the bill, therefore, you pay 25% 25% of 1567 = .25(1567) = 391.75 so I think you pay 200 + 391.75 = $ 591.75 You might better get a second opinion
