If you are talking about Alternating Current, the mechanism that is driving this is a changing electric field that is accelerating and decelerating charges within the medium. It is more like a series of infinite springs pushing back and forth against each other where some places taking on maximums and some taking on minimums depending on the direction of the forces and their location on the medium.

The charges that are changing don't themselves travel the length of the medium, they mostly act locally in space and influence upstream charges via electric field interactions in a serial fashion.

only electrons flow but the positrons are moving in the opposite direction everytime the current is altered.
