I don't know exactly what you mean by journal, but you can Google and I'm pretty sure there will be something there.  I'm so sorry I can't really help.

I'm not sure how long it is supposed to be but typically I would say: 
For your introductory, write a summary. Then for your first, second and third paragraph, say your thoughts, make them as insightful as possible. Give examples from the story. Example: I think Donald is mean because on the 4th page he calls Daisy ugly. Conclude your journal by summarizing your thoughts.
