As you know, every equation of the form \(ax+by+cz=d\) (up to a constant multple) represents a unique plane. if it contains \((1,2,1)\) then \(x=1,y=2,z=1\) must satisfy the equation:$$ax+by+cz=d\\a+2b+c=d$$same for \((2,1,4)\):$$ax+by+cz=2\\2a+b+4c=d$$
