The Quran is the central religious text of Islam. Muslims believed it to be the revelation of God. And to answer your second question, First look at when it was written. There are two theories to it, that Muhammaed compiled the quran throughout his life, or that the task of writing it down started with the first Caliph. It was recorded throughout history basically through the words of Muhammaed.

The second one is difficult to answer.. There are many theories as to how. Because one was a reciting, and then one was a compilation
